Method of payment: Payment due at services rendered. credit card, cash, check or etransfer are acceptable.
*$30.00 charge for NSF charge
Please bring any relative copies of MRI, CT scan results or any other copies of diagnostic results that could help the therapist determine your treatment plan better. The case history will need to be filled out prior to arrival fir your first massage.
If you are delayed in your arrival be prepared to have that time taken off your appointment time due to fairness in other client's appointment times. You will still be responsible to pay allotted scheduled time.
Cancellation Policy:
*24 Hours notice required to cancel your appointment. Missed/ No show appointment charge is 50 % of appointment.
If you feel ill, please stay home and cancel as soon as you do not feel well. This will not reflect a cancellation charge.
This time has been reserved for you and if you cannot make it please call to reschedule otherwise it affects 3 people: you, me and someone else that could use that appointment. Please be considerate and call if you are unable to keep appointment.
